Epic and Unique gear uses a much higher upgrade cost than Common or Superior gear, and weapons are one of the most expensive equipment slots to improve. The rarity cost factor and the weapon slot factor are both already included in the totals above.

Weapons gain level-scaled damage and armor gains level-scaled Toughness as the item level rises. Rarity and effects stay the same — this calculator prices the level change only.
The same level range costs very different amounts depending on whether the item is Common or Unique, and whether it is a weapon or a ring. Those factors are already included above.

No. Equipment upgrades use Denarii. The current upgrade system does not require a separate list of upgrade materials.
Your progression can limit the highest level currently available for an upgrade. If the game stops below your selected target, check your current level cap. This calculator uses Lv50 by default and lets you lower the cap to match your progression.
Yes. Higher-rarity equipment is significantly more expensive to upgrade. Epic and Unique gear have much higher upgrade costs than Common or Superior gear.
Yes. Weapons use one of the highest equipment-slot cost factors. Rings and amulets are considerably cheaper at the same level and rarity.
The selected range is rounded as one upgrade. Buying each level in a separate transaction can occasionally differ by 1 Denarii because each transaction is rounded on its own.
